Job Summary:

Under supervision of a Licensed Clinical Social Worker, provides integrated and coordinated case management services for three specialized behavioral health integrated services teams: Developmental Pediatrics and Autism, Gender Health, and Eating Disorders. Responsible for assisting with referrals to these departments and follow-up case management tasks to support the specialized work of these teams, including directly interacting with patients and families.


Essential Responsibilities:


  • Process incoming referrals to Developmental Pediatrics and Autism, Gender Health, and Eating Disorders following standardized workflows and using electronic medical system work queues

  • Process incoming reports for members diagnosed with Autism Spectrum Disorder, summarizing these reports in the chart and working with the family and care team on navigating recommendations for next steps in the patients care

  • Support transgender members with navigating the gender-affirming medical process by supporting the process of reviewing pre-treatment WPATH letters, tracking their care and outreaching at key treatment junctions

  • Engage with high risk, high utilizing behavioral health patients within the Integrated Services population

  • Evaluate the goals and needs of patients from targeted treatment plans.

  • Execute on tasks identified in the individualized clinical care plan.

  • Routinely assess for psychosocial gaps.

  • Navigate and advocate for patient to clinically appropriate and medically necessary levels of care.

  • Coordinate, provide and/or co-facilitate informational classes, education and/or training in areas such as adjusting to a child's autism diagnosis, parenting a child with an eating disorder, and other Integrated Services psychosocial educational topics

  • Assist with member activities of daily living as they apply to the individual care plan.

  • Work and communicate with three multi-disciplinary teams to manage care.

  • Forms completion and management.

  • Utilization of clinical and electronic tools within the EMR.

  • Provide assessment and evaluation of clinical status, environmental resources, mental and intellectual capabilities, and networking skills during the intervention period.

  • Plan services based on assessment of both short and long-term needs.

  • Track referrals and outcomes for ongoing quality management.

  • Facilitate collaboration between Primary Care, Behavioral Health Care Integrated Services and Health Care Providers

  • Provide referrals to other departments; facilitate this process and may provide follow-up services as appropriate.

  • Consult with families and other caregivers.

  • Provide service information, educational materials, and resource information.

  • Facilitate scheduling of multi-disciplinary assessment team and may participate in assessment.

  • Linkage of member with external and other community-based resources

  • Establish linkages and maintain working relationships with outside resources.

  • Provides crisis intervention as needed.

  • Consistently supports compliance and the Principles of Responsibility (Kaiser Permanente's Code of Conduct) by maintaining the privacy and confidentiality of information, protecting the assets of the organization, acting with ethics and integrity, reporting non-compliance, and adhering to applicable federal, state and local laws and regulations, accreditation and licenser requirements (if applicable), and Kaiser Permanente's policies and procedures.

  • In addition to defined technical requirements, accountable for consistently demonstrating service behaviors and principles defined by the Kaiser Permanente Service Quality Credo, the KP Mission as well as specific departmental/organizational initiatives.

  • Also accountable for consistently demonstrating the knowledge, skills, abilities, and behaviors necessary to provide superior and culturally sensitive service to each other, to our members, and to purchasers, contracted providers and vendors.

  • Marriage and Family Therapist Candidate (Colorado) OR License Professional Counselor Candidate (Colorado) OR Licensed Social Worker (Colorado)



Additional Requirements:


  • Once hired as an Associate, an employee will be offered and must work a schedule that would accumulate at least half the required supervisory hours annually.

  • Must be familiar with DSM-V-TR as a means of diagnosis.

  • Is gender-affirming.

  • Excellent interpersonal and communication skills, including as part of a multi-disciplinary team.

  • Knowledge of social service agencies, state regulations, and professional board standards as is related to member treatment, patient rights, and member/patient confidentiality.

  • Knowledge of Evidence-Based Practice.

  • Knowledge of the bio-psycho-social functions that contribute to mental health.

  • Professional maturity and ethical integrity necessary for assuming professional responsibilities.

  • Commitment to quality of service, teamwork, and participation in a highly interactive multidisciplinary clinic.

  • Ability to complete multiple tasks/objectives in a timely manner.

  • Efficient documentation skills.

  • Must be able to work in a Labor/Management Partnership environment.

  • The union and the company are currently investigating the bargaining unit status of this position.

  • If it is determined that this is a bargaining unit position, bargaining unit status (seniority dates, dues payments, etc.) will be applied on a retroactive basis based on the employment commencement date.

  • Intention to pursue licensure and completion within 3 years of hire.
