Licensed Practical Nurse - Urgent Care (Per Diem Silverdale)

Playing a critical role on our health care team, our Licensed Practical Nurses (LPNs) support clinicians and other team members while performing nursing duties and additional administrative tasks. You’ll work directly with patients or speak with them on the phone or online to help meet their needs and contribute to a positive care experience for all. You’ll also provide nursing assessments to triage patients based on their health conditions.

Description:
Job Summary:

Provide support functions to providers by gathering records and other information and facilitating patient appointments. Document appointments and procedures and follow up with provider and patient. Provide Infusion Therapy to Kaiser Foundation Health Plan of Washington members including blood transfusions, IVIG, hydration, antibiotics, etc.  Facilitate and coordinate care for members with oncologic diagnoses.



Essential Responsibilities:


  • Obtain medical record and pertinent information prior to patient visit. Escort patients to exam rooms, gather preliminary data and assist provider as needed. Anticipate providers needs according to information available. Gather information from patients and relate problems to physician. Communicates and relay health information to patients and health professionals by phone or in person as directed by provider. Relays information and communicates instructions as directed by provider. Arranges for care with provider and other resources as directed by provider. Facilitates referrals and appointments as directed by provider.

  • Performs sterile techniques, set up and assist with various procedures, apply and remove dressings.

  • Track, record and complete documentation from patients visits. Provides accurate documentation in medical records. Maintain updated flow sheets. Document all procedures and observations. Verify that coding is accurate on all lab orders according to diagnosis all lab orders according to diagnosis as indicated by provider. Maintain log books and patient tracking/notification systems. Notify patient of results according to provider instructions. Follow-up when results are not obtained or patient has not returned.

  • Initiates peripheral IVs & access central lines per protocol, administration of meds, (injectable, po and IV) EXCLUDING ANTINEOPLASTIC AGENTS agents according to provider orders and under direct supervision of Provider and RN as provided in Washington State LPN Scope of Practice.

  • Maintain equipment and supplies in exam rooms, infusion and team areas.

Basic Qualifications:
Experience

  • Minimum two (2) years of experience in a back office multi-physician office.
  • Per the National Agreement, current KP Coalition employees have this experience requirement waived.
Education

  • High School Diploma OR General Education Development (GED) required.
License, Certification, Registration
  • Practical Nurse License (Washington) required at hire OR Compact License: Practical Nurse required at hire

  • N/A
Preferred Qualifications:

  • Two (2) years of experience in infusion/oncology.
  • IV considerations.
  • Associates degree.
