Description:
Job Summary:

The Surgical Care Program Manager plays an integral role in constructing surgical pathways, standardization of care, provision of patient education, and evaluation of patient and program outcomes.  Processes across the continuum of care are routinely examined and revised, so that the most effective, safe and patient centered care can be provided.  The Program Manager is involved in all aspects of care from the moment a surgical candidate is identified through 6 weeks post-operatively.

Essential Responsibilities:


  • Actively engages patients and their families in care planning from the preoperative surgical appointment through 6 weeks postoperatively.

  • Assesses and stratities patients to ensure the most appropriate and beneficial individualized care plan can be implemented

  • Serves as central point of contact throughout for the patient, their family or caregiver, and continuum care providers

  • Handles phone triage for postoperative patients experiencing concerns with surgical complications, including after hours and weekends

  • Facilitates post discharge communication between patients, outpatient care providers and surgical team

  • Provides patient education at preoperative classes.

  • Streamlines hospitalization, discharge, and post-acute care processes for surgeries

  • Establishes standardized interdisciplinary care protocols to minimize variation in care and processes.

  • Oversees care delivery of entire team to ensure adherence to established best practices and standardized processes (Surgeon, PA, PT, RN, Anesthesia).

  • Directs collaboration with regional ERAS leadership and partnering sites for continued development and redesign of surgical protocols.

  • Works directly with OIM and regional health education to evaluate and redesign program materials

  • Designs, implements, and revises the surgical preoperative patient education classes

  • Evaluates program success through patient interviews, retrospective chart reviews, and data analysis.

  • Leads site visits to share best practices with other KP medical centers in NCAL, Washington and Oregon.

  • Manages communication and transitioning of care from clinic to hospital, through hospital discharge and throughout the continuum to eliminate communication gaps.

  • Establishes and oversees the pathway for patients going from hospital or Skilled Nursing Facility to Home Health.

  • Contacts continuum leadership to resolve patient care barriers experienced during transitions.

  • Autonomously meets with continuum leaders on ad hoc basis to revise existing processes in order to improve workflows for our patients.
Basic Qualifications:
Experience


  • Minimum of two (2) years experience in discharge planning or case management

  • Minimum of three (3) years of RN patient care experience required.

  • Minimum of three (3) years of teaching, program planning experience and adult learning theory required.

  • Minimum of two (2) years of experience providing patient education in an inpatient or outpatient setting required.

Education

  • Bachelors degree in nursing or health related field required, OR four (4) years of experience in a directly related field.
  • High School Diploma or General Education Development (GED) required.
  • Graduate of an accredited school of nursing.
License, Certification, Registration
  • Registered Nurse License (California)
  • Basic Life Support

  • Minimum of two (2) years clinical specialization in surgical area of practice.
