Description:
Job Summary:

The Eating Disorders Case Manager is responsible for the coordination of patient care services within the regional Eating Disorders (ED) program.


Essential Responsibilities:

  • Coordinates initial triage of all patients referred to the regional ED program. Gather necessary information, consults as needed, and determines appropriate treatment setting for the patient (e.g. Primary Care, ED program, Mental Health, day treatment, inpatient).
  • Orients new patients entering into the ED program. Assists patient and family in completion of forms and contract, educates patient and family about the ED program and importance of family commitment, and schedules patient and family for initial appointment.
  • Participates in ED team clinical consultations, acting as a resource for the clinical team in any needed coordination of services.
  • Outreaches to patients and families who fail to keep appointments as scheduled, assessing barriers, stressing important of attendance and commitment, and re-appointing the patient and family when appropriate.
  • Act as liaison between ED program and day treatment and inpatient care settings for patients who are admitted to the ED program who will likely be admitted post-discharge.
  • Facilitates the discharge of patients from the ED program to other treatment settings (e.g. Primary Care, Mental Health).
  • Assists the ED clinical team in measuring and evaluating quality and effectiveness of care and service in the ED program.
Basic Qualifications:

Experience


  • Minimum six (6) months of experience working with patients with eating disorders or a minimum one (1) year of experience working with patient populations with complex medical and psycho-social diagnosis.


Education


  • Bachelors degree in nursing OR four (4) years of experience in a directly related field.

  • High School Diploma or General Education Development (GED) required.


License, Certification, Registration


  • This job requires credentials from multiple states. Credentials from the primary work state are required at hire. Additional Credentials from the secondary work state(s) are required post hire.



  • Registered Nurse License (Washington) within 6 months of hire OR Compact License: Registered Nurse within 6 months of hire



  • Registered Nurse License (Oregon) within 6 months of hire


Additional Requirements:


  • Case management and or project management experience.

  • Excellent written and verbal communication skills.

  • Excellent problem solving skills.

  • Comfort with autonomy and ambiguity.

  • Strong organizational and creativity skills.

  • Strong team player.


Preferred Qualifications:


  • Minimum two (2) years of experience working with patients with eating disorders.

  • Experience in outpatient and acute inpatient settings.

  • Case management experience within a managed care environment.

  • Bachelors degree in nursing.

  • Extensive knowledge of treatment for eating disorders.

  • Knowledge of KP and organizational resources.
