Description:
Job Summary:

Plans, directs, manages and oversees clinical service delivery and business performance within KPs Home Infusion program, including those provided by contracted providers, ensuring compliance with all regulatory, accreditation, licensure and certification requirements and meets the goals and missions of the organization. Assures the provision of high quality, reputable, and cost-effective clinical services meeting regulatory requirements through supervision and management of multidisciplinary clinical and support staff (licensed and non-licensed). Has full supervisory authority for staff and associated budget.

Essential Responsibilities:


  • Manages the delivery of quality clinical services consistent with agency policy, applicable state practice acts and applicable state and federal regulations through coordination of patient and staff schedules, provision of advice to staff and patients and directing utilization of program resources (clinical and non-labor). Manages day to day operations management in alignment with the departments strategic plan departments strategic plan.

  • Hires, orients, trains, develops and supervises qualified and competent Home Infusion staff, including recruitment, selection, evaluation, coaching, rewards/recognition and discipline.

  • Assures the delivery of cost-effective care through budget development and oversight, process monitoring and improvement, individual staff performance and effective utilization of non-labor resources (outside referrals, supplies, DME, etc.).

  • Plans, organizes, directs and evaluates Home Infusion clinical service delivery to continuously improve the quality and coordination of clinical care provided to patients and families. Assures the delivery of quality homecare in partnership with the Director of Home Health and Home Infusion.

  • Directly and through subordinates, plans, organizes, directs, and controls the operation of assigned areas (e.g. general administration, patient care, professional services and/or a combination of these and other services). Contributes toward the overall development of programs and services to provide for the needs of members, physicians and staff.  Provides strategic leadership to management to ensure equitable distribution of resources and delivery of high quality cost effective healthcare and efficient management of staff in accordance with goals, objectives, policies, and applicable regulatory agency rules and regulations. Co-leads unit-based teams (UBTs). Ensures the UBTs continuously focus on identifying and implementing ways to meet/exceed quality, service, and access targets and implement regional initiatives.

  • Assures the delivery of quality homecare in partnership with the Director of Home Health and Home Infusion. Collaborates with the Management and staff in the Home Infusion pharmacy to achieve desired KPNW outcomes in Affordability, Service, Quality and Best place to work metrics. Supervises staff and manages the Home enteral and TPN Dietitian program for KPNW.  Collaborates with colleagues in Ambulatory setting to support the delivery of this RD care in the ambulatory setting for Radiations/Medical Oncology and ALS Clinics.

  • Collaborates with physicians, managers and professional staff in researching best practices, performance and strategies for achieving program excellence.

  • Establishes, maintains and utilizes effective working relationships with peers, direct reports, managers, labor and physician partners to effectively improve the delivery of quality, efficient and compliant clinical services.

  • Assures ongoing professional development for self and staff.

  • Assures home health, hospice and home infusion patients and families, at home and in facility settings, receive high quality care, through organizing and supervising clinical staff and providing after-hours administrative call.

  • Serves as the clinical expert for Infusion and Vascular Access practices for the KPNW Region in partnership with Hospital and Ambulatory leadership.

  • Serves as a liaison with the Regional and National Home Health and Hospice nursing leaders to maintain best practice, manage overall licensed nurse training and competency through development of Regional and departmental Policies.

  • Facilitates the Regional Infusion Therapy Steering Committee (RITSC)

  • Participates in community relations activities to enhance the image and reputation of the organization in local customer service areas. May represent the organization in activities involving leaders in business, government, labor, the community at large, Health Plan Members and healthcare providers in the area.

Basic Qualifications:

Experience


  • Minimum three (3) years of experience in critical care, oncology, chronic disease management, pain management or home health care.

  • Minimum three (3) years of management experience as a director, manager, supervisor or other leadership role, such as lead or program/project management experience or related consulting experience where responsibilities would include managing teams of people, timelines, budget, etc.

  • Minimum three (3) years of progressive nursing leadership experience in the last five (5) years


Education


  • Bachelors degree or higher in nursing


License, Certification, Registration


  • This job requires credentials from multiple states. Credentials from the primary work state are required before hire. Additional Credentials from the secondary work state(s) are required post hire.



  • Registered Nurse License (Washington) within 6 months of hire OR Compact License: Registered Nurse within 6 months of hire



  • Registered Nurse License (Oregon) within 6 months of hire



  • Drivers License (in location where applicable)



  • Basic Life Support


Additional Requirements:


  • Thorough knowledge of OASIS dataset

  • Thorough knowledge of Home Health Medicare Conditions of Participation, state licensure regulations, and Joint Commission accreditation standards

  • Ability to effectively utilize computerized medical records

  • Proficiency in the use of applicable computer software (e.g., MSWord, Excel, Outlook)

  • Willingness to work in a Labor/Management Partnership environment


Preferred Qualifications:


  • Minimum five (5) years of supervisory and management experience

  • Minimum five (5) years of direct experience in hospital or healthcare services field

  • Minimum two (2) years of Home Health/Hospice supervisory experience.

  • Nursing leadership experience

  • Demonstrated ability to collaborate with other clinical departments to leverage best practices.

  • Demonstrates customer-focused service skills

  • Knowledge of continuous quality improvement principles

  • Excellent written and verbal communication skills; analytical problem- solving skills; well-developed project management skills; competent quantitative skills

  • Masters degree in nursing.
