Description:
Performs inpatient chart analysis for medical record documentation requirements, notifies surgeons of status of outstanding surgical reports, provides statistical support for the timely and accurate completion of inpatient and ambulatory medical records, and notifies physicians of the status of their incomplete records. Accountable for ensuring all special requests for charts for auditing, billing and review are performed in a timely manner.
Job Summary:
Performs inpatient chart analysis for medical record documentation requirements, notifies surgeons of status of outstanding surgical reports, provides statistical support for the timely and accurate completion of inpatient and ambulatory medical records, and notifies physicians of the status of their incomplete records. Accountable for ensuring all special requests for charts for auditing, billing and review are performed in a timely manner.
Essential Responsibilities:
- Analyzes charts of multiple facilities for required documents and signatures from each clinician that participated in the care of the patient during the encounter. Monitors reports in EMR to identify delinquent charts and prioritize and ensure physician completion and removal of delinquency from the Chart Deficiency Tracking System. Communicate with physicians by all available means to facilitate completion of chart prior to delinquency date.
- Processes newborn information into the Electronic Birth Registration System, maintains confidentiality for adoptions and handling of records considered as privileged. Assists Funeral Homes, with completion of Death Certificates for our patients.
- Retrieve and Process Hospital ECT encounters. Prints Daily Hospital ECT census sheets, to complete the tracking and reporting process, before sending the required information to the Outpatient Medical Records for scanning into HealthConnect.
- Answering department phones and responding to all department requests. Filing and Faxing of documents. Provide technical and administrative assistance for other health information services functions. Performs all other duties as assigned.
Basic Qualifications:
Experience
- Minimum one (1) year in compiling hospital chart deficiency data.
- Minimum six (6) months experience with the EHR - Electronic Health Record.
Education
- High School Diploma or General Education Development (GED) required.
License, Certification, Registration
- N/A
Additional Requirements:
- Working knowledge of chart deficiency automated systems.
- Demonstrated ability to communicate in an effective manner, verbally and written.
- Demonstrated ability to easily adapt to unfamiliar automated systems and office equipment.
Preferred Qualifications:
- Minimum two (2) years of experience in compiling hospital chart deficiency data, working knowledge of the EHR - Electronic Health Record.
- Completed basic training in automated systems and a working knowledge of chart deficiency automated systems in a health maintenance organization structure.
- Completed training in Outlook and Teams systems
- RHIT
- Ability to identify potential problems, visualizes a resolution, and effect a change through positive communication.

Pay Range: $39100 - $50600 / hour
