Description:
Job Summary:

Provides variety of ambulatory behavioral health services including evaluation, diagnosis, treatment planning, medication management, case management, and psychotherapeutic intervention to individuals, families, groups and/or Intensive Outpatient Programs. Provides services to adults of all ages, adolescents, and children as within the scope and licensure/certification.

Essential Responsibilities:

  • As a credentialed and privileged provider, in collaboration with physicians and other behavioral health providers, manages care for patients with behavioral health conditions including assessment, evaluation, education & development of individualized plan of care including medication management. Reviews and establishes diagnoses. Orders and monitors diagnostic tests, performs procedures as within the scope of practice.
  • Utilizing the latest Diagnostic and Statistical manual of Mental Disorders (DSM) and International Statistical Classification of Disease and Related Health Problems (ICD) for diagnostic references. Be familiar and utilizes a wide range of the latest evidenced based interventions and modalities.
  • Provides psychotherapy and counseling for individuals, families, groups and/or Intensive Outpatient Programs. Utilizes a wide range of intervention modalities, which support positive mental health and enhanced individual or family functioning. Provides services such as medication management, crisis intervention, brief therapy, individual, group and family therapy. Interviews member and evaluates their psychological and mental status. Refers members in need of immediate or urgent care to appropriate level of services, within time frame established by policy. Collaborates with physicians, primary care staff, and other interdisciplinary clinical team members involved with assessment and care of patient. Follows program description in area of specialty as applicable.
  • Researches, collects, and records data and information relating to identification and treatment of behavioral and interpersonal problems of patient. Formulates treatment plan for these conditions. Assists to establish clinical best practices and guidelines for delivery of service to population of patients served.
  • Documents clinical assessments, treatment plans, interventions, and other relevant information in patient medical record according to regional and departmental guidelines. Maintains confidentiality of all patient information and records in accordance with state and federal regulations and regional guidelines.
  • May perform patient care to the extent necessary to maintain clinical expertise, competency, and licensing necessary to fulfill job responsibilities and to direct the provision of care on the unit.
  • Provides direct patient care. Provides services that are within scope of license and in compliance with all legal, regulatory, and policy requirements relevant to clinical role performed.
  • Incorporates the KP Nursing Vision, Model and Values throughout their Nursing Practice.
Basic Qualifications:

Experience


  • Minimum two (2) years experience in mental health managing patients on psychiatric medications.

  • Minimum two (2) years experience in mental health providing direct clinical services with children and/or adults, including people with serious/chronic mental illness.


Education


  • Masters degree in Nursing with specialty in community mental health, behavioral health, psychiatric nursing, or related field.


License, Certification, Registration


  • Psychiatric-Mental Health Clinical Nurse Specialist Certification OR Psychiatric-Mental Health Nurse Practitioner Certification



  • Advanced Practice Registered Nurse License with Prescriptive Authority (Hawaii)



  • Registered Nurse License (Hawaii) required at hire



  • National Provider Identifier



  • Basic Life Support from American Heart Association


Additional Requirements:


  • Demonstrated knowledge of assessment using the latest Diagnostic and Statistical Manual (DSM) and International Statistical Classification of Disease and Related Health Problems (ICD), writing treatment plans, including medication management for patients with behavioral health conditions.

  • Demonstrated knowledge and/or experience of working in an integrated behavioral health model within a primary care setting.

  • Taxonomy code required at time of hire.

  • Demonstrated knowledge of documentation according to national accreditation guidelines.
