Essential Responsibilities:
- Maintains quality of care in individual programs by developing, managing and reporting quality initiatives, designation of quality measurements and regulatory reporting of quality outcomes.
- Serves as a liaison between patients, physicians, regulatory bodies, vendors, and support departments (e.g.: DME, pharmacy, laboratory, quality management, utilization review, member relations).
- Serves as primary representative of the renal program in the Northwest Region in partnership with assigned nephrologist.
- Research the Nephrology healthcare environment for innovative concepts and best practices, assimilate and comprehend the evidence, and develop practical applications for the program.
- Responsible for planning and implementation of program activities. Takes independent action and accountability for achieving successful results.
- Assess and manage patient response to renal medications, treatments, and interventions associated with the individual renal plan in collaboration with the nephrologist.
- Follow prescribed protocols. Provide ongoing education to patients associated with the individual renal plan.
- Document all relevant data including physician orders, lab results, vital signs, treatment parameters, patient status, with emphasis on lap slips, referrals, authorizations, and any treatment data related to the individual renal program and enter into the appropriate medical information system.
- Rotate coverage with other licensed staff to ensure reliable and adequate coverage.
- Collaborate with community partners, (dialysis and transplant centers, vascular access partners) and attend meetings as required
- Perform other related duties as directed.
- Minimum three (3) years dedicated nephrology nursing experience within last five (5) years (chronic kidney disease, hemodialysis, peritoneal dialysis, home hemodialysis, and/or renal transplant).
- Bachelors degree in nursing four (4) years of experience in a directly related field working with renal patients.
- This job requires credentials from multiple states. Credentials from the primary work state are required at hire. Additional Credentials from the secondary work state(s) are required post hire.
- Registered Nurse License (Oregon) within 6 months of hire
- Registered Nurse License (Washington) within 6 months of hire
- Basic Life Support
- Nephrology Certification (Certified Nephrology Nurse or Certified Dialysis Nurse) upon hire, within 12 months of hire, or upon eligibility.
- Excellent written/verbal communications; interpersonal skills.
- Demonstrated ability to work with multi-disciplinary team within complex organizational structure.
- Able to work independently in an unstructured environment with minimal supervision.
- Computer proficiency (preferably in Microsoft Windows environment) to effectively communicate, gather data, and generate reports.
- Willingness to learn computerized information systems.
- Ability to present reports/presentations verbally in a public setting (public speaking).
- Excellent customer-focused service skills.
- Experience providing patient education and / or training.
- Knowledge of adult learning theories and models.
- Will be required to travel to service areas throughout the Region.
- Must have reliable transportation.
- Strong written and oral communication skills.
- Coordination of patient care within a managed care system.
- Demonstrates clinical nursing and leadership skills.
- Experience in delegation of patients care or case management in a managed care environment.
- Knowledge of the care needs of patients with chronic conditions including diabetes mellitus, hypertension and chronic kidney disease.
- Demonstrated ability to coach and lead others.
- Strong conflict resolution skills and the ability to develop and maintain good working relationships.
- Experience in database management.
- Demonstrated knowledge of pertinent regulatory, accrediting and advisory body requirements, including but not limited to county, federal and state regulations.
- Experience with adult education.
- Ability to adapt to constantly changing priorities and managing a wide range of projects.
- Strong facilitation skills.
- Must be able to work in a Labor and Management partnership environment.
- Membership and participation in professional nursing organizations relevant to role.
- Experience in conducting telephone assessment of patients.
- Knowledge of five stages of chronic kidney disease.
- Demonstrated effective interpersonal, communication, and problem-solving skills.
- Demonstrated ability to work within teams and within a dynamic work environment.
- Demonstrated customer-focused service skills.
- Leadership and/or case management experience.
- Knowledge of case management principles.
